To submit your nomination, please complete this official entry form and attach an accompanying narrative for each submission describing the nominee’s activities and/or contributions that warrant consideration for each award. You may also type the narrative in the space allocated on this form.

All nominations must be received in the RPA Office by January 9, 2009.
Applications received after the deadline will not be considered.

Narrative describing the nominee's activities and/or contributions that warrant consideration for this award. If narrative text is over 255 characters - please use the upload option below.

Narrative Upload Option:

Have word doc or pdf saved on your computer.
Click the Browse... button
Browse and select your document via the pop up window.
Once you highlight/select the filename - the filename should now be displayed in the pop up window's File Name box. Make note of this file name - you will need to enter it in the next field.
Click Open - the pop up window will disappear and the file to be uploaded will now appear in the field next to the Browse button.
In the next field - File name of uploaded document - type in the filename only of the document you just selected.
Once all data has been entered click submit. This will upload the document for submission with this application.
