    Advanced CKD Patient Management Toolkit

How to Utilize RPA’s Advanced CKD Patient Management Toolkit

    CKD Guidelines

Clinical Practice Guideline on Appropriate Patient Preparation for Renal Replacement Therapy
The patient population at the center of RPA's third clinical practice guideline is the subset of adult patients (18 years of age or older) who have advanced chronic kidney disease or a glomerular filtration rate of less than or equal to 30mL/min/1.73m2 when kidney function is at a high risk of progression. The recommendations contained in the guideline are intended to provide clinicians with practical guidance for the care of individuals with advanced CKD not yet requiring renal replacement therapy.

    Position Papers on CKD

RPA Position on Development of Effective Collaborative Practice Models for Chronic Renal Care
RPA believes that appropriate use of collaborative practice models in renal care represents an effective method of providing high quality care to a patient population experiencing vast growth in an era of diminishing nephrologist resources, and also has the potential to simultaneously
improve the quality of patient care delivered and provide greater costefficiencies in the delivery of that care.

    Vascular Access

    Vascular Access Initiative
RPA and major dialysis organizations nationwide have joined forces to develop and launch the Vascular Access Initiative (VAI), a collaborative project to improve the unacceptable rate of central venous catheter (CVC) use in the nation’s kidney patients receiving life-saving hemodialysis. This series of documents and tools is intended to facilitate that effort.

    Additional CKD Resources

    NKDEP
The National Kidney Disease Education Program (NKDEP) provides free tools and resources for identifying and managing CKD patients as well as patient education materials and tools in English and Spanish. Established in 2000 by the National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK), NKDEP’s chief objective is to reduce the morbidity and mortality caused by CKD and its complications. RPA regularly collaborates with NKDEP.

    American Kidney Fund
Helping Patients Prepare for Kidney Replacement Therapy - This course for physicians, nurse practitioners, physician assistants, and social workers addresses kidney replacement therapy and dialysis access options for patients with stage IV and stage V kidney disease.

    Kidney Disease Education
Implementation of Kidney Disease Education - the  NKF and RPA Partnership for MIPPA Implementation of KDE Benefit have developed resources for providing kidney disease education, including:
